Forms & Informed Consent

MyCounselor is a provider of “Mental Health Services”. As such we must collect some information about you and your presenting concerns for our records (Intake Forms). We also need to provide you with some information about the services you will receive (Informed Consent). You’ll need to complete each of the following documents before your first session. Completing the documents prior to your first session is very important, as we can not provide you with services without these documents complete. If you have any questions please give us a call at 855-593-4357 and we will be happy to answer them.

Can I use my insurance?

Counseling is a covered benefit under many insurance plans and some clients have received out-of-network benefits for our services.

Some networks, recognizing our excellence in certain specialized areas, have agreed to reimburse our clients at in-network levels even though we are not on their in-network panels. This is solely at the discretion of your insurance provider. Services offered through us are generally not covered by Medicare or Medicaid.

How do live video sessions work?

In video sessions with your counselor, you see each other and talk with each other in a virtual face-to-face setting, in our case it is a HIPPA Compliant Zoom Room.

At the time of your schedule your appointment, you will click the link sent in your appointment confirmation email. That link will be redirected you to a “digital waiting room” where you wait until your counselor lets you into the digital meeting room.

How long/often are meetings with my counselor?

The exact frequency you meet with your counselor is unique to you and your situation. On average clients meet with their counselor 2-3 times a month for 45-minute meetings

Some people prefer to meet less frequently or for longer 90-minute meetings. You will choose meeting frequency and duration based on your counselor’s recommendation and your preferences.

Can I use my FSA/HSA for counseling?

Yes.

To use your FSA/HSA with us, all you need to do is provide your card details when you meet with your matching specialist, just as you would for a normal credit card.

Using HSA or FSA dollars for MyCounselor online counseling is a great way to save money — with these accounts, you’re using pre-taxed income you’ve set aside for health care costs.

How do I cancel appointments?

You can use the cancellation link in your confirmation email or contact Member Support. You will be responsible for paying the full session fee if changes are made after the 24-hour mark prior to the appointment in question.

Clients who experience an internet outage or other technical difficulties are encouraged to attend their session over the phone.

How can I stop my subscription?

Cancelling your subscription can be easily done online. You can cancel future subscription payments online by scroll up a little bit and clicking on “Membership Cancellation Form” under the “Additional Forms” column.

You’ll know that your membership has been ended when you receive the confirmation email with the subject “Goodbye for now”.
